Delete record from database using radio box

You must excuse me iF this question seems overly trival/basic, but i am new to asp programming.
The query:
I have a radio button on a form that when selected will delete the username from the database. How can i send the request from the form to then be executed by the asp script (e.g delete.asp)?
Any quick fix would be greatly appreciated
