I was wondering if someone can help me!

I need to design a web site for a telephone directory.

This will be a self-sustained database driven site where people can 

register their details on-line.

It will consist of a:



Individual Register Form including: 



Name 

Surname 

Physical Address - Street, Area, City, Code 

Postal Address 

E-mail address 

Website URL 

Telephone No. 

Fax No.

Business Register Form including

Business Name 

Industry Type 

Physical Address 

Postal Address 

E-Mail address 

Website URL 

Telephone No. 

Fax No.

The database must have a tick-box for me to tick 'YES' once payment has 

been received from the Register.



There must be a Search Form that searches individually or a combination of 

2 or more of the following:



Alphabet 

Name / Part thereof (I.e.. cou for Cousins) 

Surname / Part thereof 

Street Address 

Area 

City 

Industry Type 

Tel. No. 

Fax No.

Eg. I'm looking for a Plumber in  (Area), (City)...

So it will consist of drop down menus, tick boxes, etc.





I need code to allow the above mentioned data the user submits to be 

stored in a database.

Then I also need code that allows the user to search the database for the 

above mentioned topics.

Is there any code that can help me?



Please advise!  My first job depends on it.
