I think that the images folder at:
http://localhost/ThePhile/Modules/Lists/Admin/Images/
is missing. I have imported most of the images that should be in this
folder from other modules, but a few are missing.
Might want to change the download zip if this folder is missing.

Hi Kevin,
you are correct that the following folder is missing:
/ThePhile/Modules/Lists/Admin/Images/
I have found a copy of all the images in this directory:
/ThePhile/Modules/MailingLists/Admin/Images/
simply copy the directory over.
