I'm experiencing the same this now.  Did anyone figure this one out?  

Figured it out.  Thought I'd post it for future generations.  Forms-based 
authentication requires an entry in web.config.  I read about it in Jeff 
Prosise's book, and Wrox's Professional ASP.NET.  The entry is probably in 
the code download.  The research was a good learning experience.
