    Thanks for your quick response.let me elaborate with examples for a 
single action.

A problem may be like this:
    Inranet server is very slow.

In this case the action owner will be some system administrator in that
company who could deal with that problem.
Email id id the id of that action owner.
he will take some action to rectify the defect and he will the target date
field with the an estimated date of problem solving.
Implementation date refers to the actual date of problem solving(action 
implemented date).it may be few days less or greater than the estimated
target date.at once he solves the problem,he will take that form and fill
the implemented date.


Like this there are few actions.on finishing each action he will fill the 
implemented dates corresponding to those actions.if he solves two or more
problems,he should be able to fill the corresponding date fields in single
