asp_databases thread: Discuss table setup for web form information

I have a web form with the following.

User info. fname,lname,title,idnum

then I have questions. some radio button questions,

some checkbox questions, and text area.

Should I create 1 table with columns for each

question? 

Or should I create 1 table with name/value pairs for

each question?

Or should I create 2 tables, one with colums for

unique user information and the other for name/value

pairs of the questions?

Most of answers to these questions can be determined by doing some proper

modelling before you begin. Have you done an ER model? Have you done some

software specifications (say a data flow diagram which would show you how

you need to manipulate this data)?

Ken,

Whats an ER model and where can I learn how to do

them?  I'm collecting web form data.  Some basic user

information, then different required and optional

information that they select via checkboxs and radio

buttons.  I will need to run querys based on answers.
