Chapter 12 forgot password addition

Hi

Is there a way I can combine Chapter 13's "forgot password?-option" with my site script in Chapter 12?

Please help me with this, I already tried this, but with the PHP Script layout-difference between chapters 12 and 13, I only get confused and don't know where to include which-and-what.
