I have recently come across the same problem after applying MDAC2.6 and
Post SVPK6a security update on NT4 - check out articles Q275917, Q295166,
Q310703 and Q288388. I installed the active directory client services
(dsclient.exe) and the Q310703 patch and everything worked after a re-boot.
