I recommend sticking with XHTML 1.0 Strict...

I recommend sticking with XHTML 1.0 Strict because its what Microsoft used to create their master pages. Other doctypes will work though. HTML5 has some gotcha's in SP2010 though, like in IE9...

The reference that starts with "/" takes you to the top of your web application. In your case you have a site collection that isn't at the root of the web app. So the reference should be:

I think the default initial publishing portal...

I think the default initial publishing portal page layout hides the quick launch. Try changing the page layout to like article or something.
