I am developing a documentation system for my Pharmacy Dept. The pharmacists would like something simple. I was wanting to have a form where they would simply click on the intervention type they performed and it would be added, by name, to my table.

Any suggestions on how to do this? I've tried using checkboxes but it didn't work.

I think that if u tried check boxes for intervention type, they are limited and known. U should try a data entry form with a list box relating to ur table.
