Depending on the version of Access:

Beginning Access 2000 VBA
by Robert Smith and David Sussman
Wrox Press © 2003 (864 pages)

Beginning Access 2002 VBA
by Robert Smith, Dave Sussman et al.
Wrox Press © 2003 (945 pages)

Beginning Access 2003 VBA
by Denise Gosnell
Wrox Press © 2004 (544 pages)

Beginning Access 2007 VBA
by Denise Gosnell
Wrox Press © 2007 (574 pages)

Did that help?

There is no one particular book to answer completely for any aspect of access. It depends on what areas of form design or what you are attempting to achieve, which books will be of more help than others. There is an Que book on forms however its fairly basic though has the odd useful piece in it.

In addition to the list above, which are very very good I'd add:

Building Microsoft Access Applications by John Viecas
Microsoft Press

Access Step by Step by John Viecas
Microsoft Press

Expert One to One Access Applications
Wrox Press
