Please suggest a good book which clearly explains forms with example and VBA code....

There is no one particular book to answer completely for any aspect of access. It depends on what areas of form design or what you are attempting to achieve, which books will be of more help than others. There is an Que book on forms however its fairly basic though has the odd useful piece in it.

In addition to the list above, which are very very good I'd add:
