Submit/pass form data to a word doc

I have an ASP page that accepts a user's input like their name, ph#, dept etc. I also have a hyperlink on the page that opens up a word doc. I would like to pass this info (name, ph#, dept etc) from my page to this word doc so that the user can avoid typing it twice.
Any suggestions??
