Well, I do not know what to tell you. I have used UrlRewriter.Net and another ISAPI filter that both use Regular Expressions to handle the Rewrite in all cases, the front end of the website has no idea that the rewriting is going on as my links are all Spider friendly.
I have never had a situation such as you are describing. ><
