Have you looked at Chapter 4, the Wrox Survey Engine in ASP.NET 2.0 Instant Results?
http://www.wrox.com/WileyCDA/WroxTit..._chapters.html

You can buy the whole book or a PDF of just that chapter.

Thanks. Does it include code in that chapter? or just basic study about survey?

Appreciate your great help.

Yes, I should have mentioned that. You can download the complete code for the survey module from the book's page here on Wrox.com (it comes on a CD if you buy the book). And the chapter goes through the concept and the code in thorough detail.

Thank you.

I will go B&N today and buy this book with CD.

Thanks for the help.

One last question, Is that code and help available in VB.NET or C# or both versions?
because my project was developed in ASP.NET/VB.NET. Need to add Survey module to that. Even if it is C#, there are no problems, we can convert. Just curious

You're in luck, they're VB. That should save a little work.
And one of the authors, Imar, is very active in the book's forum here:

Default Survey Module

Hi Jim,

Finally i got the book and found it useful, but not 100% and it is not solving my problem.

In that chapter, only radion button questions were mentioned. We have to generat all type of questions.

Am i correct about following ?

Generate controls based on user selection.

if selection = radio then
' chapter 4 radio button questions (this condition is currently in chapter 4)
this is what am trying to do? am i correct?
elseif selection = checkbox then
'Check boxes questions
elseif selection = single textbox question
elseif selection = rating question then
---
end if

I under'd the db but What about if add rating, single textbox questions. Do we need to change db layout? i stucked how to design db for these type questions.

Any help please!!!
