Default problem in getting the value of an array

how will i get the value of an array and display and save it value...

i have a form that has a radio button, each radio button has different job see the picture,

as you can i see the, when i select the fiirst radio button, i will be sent to another page say create.php in that page the date indicated in each array box should be displayed on the top most part of the webpage like this one...

i only edited the date in the image what i want is to display whatever the dates indicated in the array boxes...
