Great, I moved this to our PHP forum. A great place for you to learn to do this is with the w3schools tutorials:

I'd suggest starting there, seeing what you can accomplish yourself, then coming back to these forums with questions on any trouble you have. You'll get better answers here to a specific "I can't get this to work" question than a "please do this for me."
