uploading images

I work example of books PHP and MySQL 24-Hour Trainer,.
In this book I have to manually type the name of picture
( in <input type="text">) that she has been save in database.
I want to upload images in file and name of images in database using <input type="file> does anyone know how to do that.
Application work fine.
If anyone work this example from this books and know how to do that please let me know.
