Default Problem with Errata and No Forum for my book?

I have VB 2005 Express Edition Starter Kit by Andrew Parsons ISBN 0-7645-9573-3. There doesn't appear to be a forum for this book and I am having difficulty understanding what I am supposed to do to fix the issue with Chapter 7 page 126 errata. I have played around for a couple hours with different connection strings and can't make the program work right. Please help!
