On p.411 it says to enter (LocalDB)\v11.0 in the Server Name field, but I get an error when I do this. I am using Express 2015 Web instead of 2013, but I don't think this should make a difference. Ideas?

Imar, you're a genius. (localdb)\MSSQLLocalDB connects well with Express 2015 Web. I wonder if there are other issues with not using Express 2013 Web that your book is based on?
