Default File Upload in form view

I am trying to build a store and i want to upload box art but I....

a) have the paths for thumbnail and full image in the same table as my items
b) want to use a form view.

first i tried it it came back with an insert disabled for this control error. so i am not sure if the code is transplantable from a listview into a form view.

I was wondering if it's usable in a form view as i am a bit stuck on how to implement the control so it can upload to the site and write to the DB.

Exactly what is usable in a FormView?
