Default Question on adding SharePoint templates to VS

Several questions:

1. On page 49 step 1 it tells me to select SharePoint's Site Definition template. But the IDE -- it's Visual Web Developer 2010 Express -- doesn't show any SharePoint templates. Perhaps this is because the IDE was on the machine before I added any SharePoint stuff. How do I get the IDE to know about those templates?

2. When the book says Visual Studio does it mean Visual Web Developer Express? Am I using the right tools here?

3. I had to uninstall Click-to-Run to get SharePoint to install -- and that took the Office express -- or whatever it's called -- with it. Is it best to just spring for the real, full Office Professional package?

Perhaps I should get the 2013 book and use that?

Advice welcome! I'm really lost here. Thank you.
