Question Maid To Order UserID and Password???

I recently bought a digital copy of Professional ACCESS 2013 Programming. I also downloaded from the Wiley web site the 3 accompanying files that will allow me to follow along with the manual. I used my Office 365 account to load the first of the 3 files that I downloaded (Maid To Order Customer Service app). I *even* read the ReadMe notes! All of those things seemed to go well, but when I launch the app, it asks for a UserID and Password.

I don't know what they are. Can anyone help me out?
