Question which open source tools is helpful for planning a software

In the introduction of the book, Thomas Myer traced my progress into the PHP programming world (last 5 years) exactly!!! LOL!

A week worth of coding came down to two days using Codeigniter and now I want to get better at this, so I bought this book.

I desperately need to know how to plan the design of an application PROPERLY. So far its been just sketches and scibbles on a piece of paper.

I would like to know the following from Thomas Myer. (besides the tips I got from his book with regards to Agile and its implementation.)

1. Any resources on the web that shows the pencil and paper planning in a systematic way.
2. Any visual tools that I can use to help me out. A list of the open-source tools is given below.

I am about to start a project based on PHP + Codeigniter. I want to do this one properly so that if I have to look back at the code after three months, I should know what's what.
