Hi Edward. Missing pages like this are a printer/bindery problem. For all the "six sigma" quality in things like electronic devices, it's still fairly common in the printing business for a few dozen copies of a book out of every 10,000 printed to get a set of pages left out.
Anyway, if you bought your copy at a brick & mortar store, they'll gladly let you exchange the defective one for a good one. If it's inconvenient for you to return the book for an exchange, please try contacting our tech support department to arrange for a good copy.
As far as the code goes, the editors on that book are working on getting this.
