Default How to create a Add-In in VS

Hi,
    I want to create a Add-in in Visual Studio. I searched a lot of projects in www.codeproject.com, But I found few issues.
   1. When I ran those projects I was not able to see the Add-In inside the Tools button and nor in the Add-In Manager.
   2. After installing the installer for the project, I was able to see the Add-In in the addin Manager but not Inside the Tools button, where actually the addin is supposed to be.

What I want is, I just want to open a Win Form from a Add-In and then will code the Win-form according to my need.
   So If anyone is awar of the issue, please let me know, I will be really thankful to him.
