Default Add a menu option to IE menu

Hello guys, I want to add a menu option to the drop-down menu of internet explorer (IE). This is much like you see when you install a download accelerator and it ads up its menu option to the drop down
menu and associates its own functionality to it. Can anybody help me out how to accomplish this in .NET. Should i use Windows APIs or there are .NET namespaces available. Reference to any link(s) is also welcome.

I have gone through exploring the Windows APIs relevant to menus to some extent and tried to get some solution but it doesn't seem to work. I can show the code if one can help and requires it. additionally, i'll appreciate if any one will give me the link(s) about the code examples about manipulating the menus using APIs. The code can be in VB, VB.NET, C# or C++.
