Default teleconferencing

Hi,
I want to add annotations to an image in a web page which has to be shown to all the connected users who are accessing that page at that particular time,ie, a web based application using which many people can change the image (point to a particular part, add text to some parts)and at the same time see what others are doing so that they can teleconference using the web. How do I go about doing this ? Use applets or other things ? if other, what ?
