Default Using Mapquest link

Forgive me as being a newbie. I've researched all I can but cannot find the answer, so I turn to you pro's. Problem I want to link the mapquest site form, option #2, form with starting point only. I'm inserting the form in Dreamweaver and it previews fine, when I upload it to my server host, godaddy, I get the 405 error, invalid http verb? Do I have to make a folder on the server side and if so what? Don't know where to go from here? Thanks to those who contribute.
