Please who can help me with applications that i can use to create PHP pages from MySql database.

Something like PHPMaker, mine has expired and i just got into PHP and am really enjoying it.

I will need it as soon as possible , also who can send me a sample formmail in PHP that sends the contents of a form to my e-mail address and also to the database.

Don't you code PHP?
