Default AvantGo

I am trying to develop a page for the AvantGo service (www.avantgo.com). I'd like to be able to produce (using PHP) a page which allows multiple form submissions. I've looked at the AvantGo documentation but the JavaScript and perl stuff didn't make sense to me.

If anyone has any advice or experience of what I am trying to do I'd appreciate the help.
