Default Problem with a registrations/application script

now that works and my mysql login info in the script is correct and all

but it doesn't seem to save correctly

after you fill out the info on that first link

then go to

http://scoobweb.com/stuff/test/userman.php

I tried and got that:
Your details were not added due to some database problem
The second link doesn't open any windows ...
