PHP and MySQL

Hi there!

I've recently programmed a server and generated an account creation page. I'm using a program called PHPTriad to attempt to link the account page with my MySQL server, but to no avail. If you visit http://mindwow.kicks-ass.net/index.php, you'll see what my problem is.

If someone could walk me through configuring my setup to allow this to connect to the MySQL database, I'd really appreciate it.
