Default How to save Radio Button value on database.

Hi everyone,
I was assigned to make an emotional testing for my project which uses radio buttons.One radio button for Adventurous and one for Impulsive. The user should choose the best answer that describes him/her. If the user chooses Adventurous out of Impulsive it has a given value of 1 and 8, and it should be saved on the mysql database. I’m just new here and I don’t know where to start.
Your help is greatly appreciated.

u should initialize the name name of all radio as same.

selected attribute use with those which u want selected.
