Have a simple registration form, it basically works, except for the following 2 glitches:

 1. it allows blank foelds to be left as they are and inserted into the database

2. It duplicates inserts/allows to register twice with the same user name.

It must be something simple but when you start the journey, the stones seem to be mountains. Appreciate help and links to code, if possible.
