Default Exe Server without using ATL

I've got to write an EXE COM Server, without using ATL, ATL COM
Wizard etc., rather using simple C++, from the scratch. I've
written the Dll COM Server the same way, but I am facing problems
 with EXE Server.

Somebody told me, that Microsoft Press's "Inside COM" contains the
procedure for it, so if anybody can provide me link to samples of
"Inside COM", I'l be grateful, or if someone has done the same, i.e.
written Exe Server without ATL/Wizard, please let me know.

 Thanks in advance.
