Default InstallShield Error

I am using InstallShield template-one to build setup for my VC++
application.
The setup is copying the files to destination directory.

But it is not creating start programs menu entry correctly. It creates shortcut with default application ie Myapp.EXE


Can anybody suggest how to correct this error.
