Default Displaying data in a tree view from combo boxes

How do I create a tree view based on data from 2 different combo boxes and a textbox? The first combo box is the 'Level' combo box. It displays the different levels in a tree. The second combo box displays all the parent code for each level. The textbox is for user to key in the child code for each parent of each level.

All these data will be displayed in a treeview.
