import data from csv/xsl file to sql

hi there i need to import data from an csv/xsl file to sql 2005

csv file has 6 columns and sql table has 6 columns, what i need to do is add 4 digit number to the 3rd column on insertion to sql and remove 4 digits from the 6th column
