Default Opening a Template from the WEB

I have created a template that opens a form for user input, which is embedded in the body of the document. I 'show' the form in both the Document New and Document Open Actions. When I place the template in my Workgroup Templates folder it works perfectly when using the Create New in Word. However, when I place it in our Web site template folder and open it using the Create New, Templates on My Web Sites, the document opens but the form does not open (rendering the document useless).

Can someone please help??
