I have a Excel spreadsheet that has columns, rows and values. How do I set up a table in access that works with the form i have created that will populate 'Value" when 'Mode', 'Color' and 'Type' are chosen. Any and all help would be appreciated. Please see example of Excel spreadsheet and Access form attached.

I am not really sure what you are after here.

Do you want Access forms for Excel, or do you want a database based on your table?

In the second case, import your spredsheet into Access (Insert > Table > Import Table) and then create Make Table Queries based on your look up columns, and then create links to the made tables.

Then click on your main table, and click the AutoForm button, and it will recreate the form you already have with combo boxes.
