Default Transaction's to Database

Hello All,

I was wondering in an enterprise environment what should you track when a user performs CRUD operations on a database. Also what error handling routines should I implement in case something goes wrong during a transaction, i.e. the user is not able to save a record etc. I welcome any book reccomendations and or articles.
