Unfortunately, there is no "exact process". You'll need to contact your ISP (where you bought your Web space) and ask them how you should handle things.
Some ISPs allow you to use FTP. This is great when working with Dreamweaver, because in DW you can then point your Remote Server on the Site Definition dialog to your FTP account.
Other ISPs use a Web based upload system. When that's the case, you'll have to manually upload a page when you have changed it in Dreamweaver.
