Default Acrobat document with Parameters

In reading the book, a good example is given for calling a report without parameters, with one discrete parameter, with multiple discrete parameters, and a range of values. However, when it comes to generating an Acrobat file, things are fairly limited: no parameters. Can someone give me an example of generating a .pdf with one discrete parameter, with multiple discrete parameters, and a range of values. This will probably go a long way to helping a lot of people.
