i use this code snippet. you'll see what the various variable names are. grab the values using:

<%Request.ServerVariables("VARIABLE_NAME"))%>

<html>
<head>
<title>Untitled Document</title>
</head>
<body bgcolor="#FFFFFF" text="#000000">
<table border=1>
<tr>
<td>Variable Name</td>
<td>Value</td>
</tr>
<% for each key in Request.ServerVariables %>
<tr>
<td><%=key%></td>
<td>
<% if not len(Request.ServerVariables(key))>0 then
    Response.Write "&nbsp;"
else
    Response.Write Request.ServerVariables(key)
end if
Response.Write "</td>"
%>
</tr>
<% next %>
</table>
</body>
</html>
